The North Shore Health Network provides rural healthcare for communities along the north shore of Lake Huron – including services in Blind River, Thessalon and Richards Landing. The North Shore Health Network sites have been serving their communities faithfully for many years providing emergency, acute, complex continuing care, long term care and community support services.

The Role:

North Shore Health Network (NSHN) is seeking a highly motivated, self-directed individual to join our multi-disciplinary team based from our Blind River Site. Reporting to the Chief Nursing Executive you will collaborate with the team of allied health professionals, liaise with partnering agencies and oversee complex discharges from Acute Care and the Emergency Department. Within your scope of practice from a culturally safe and inclusive lens you will support effective patient centered care as you provide crisis intervention, assessment and create care plans with patients and their loved ones.

Challenges that patients may be presenting with include but are not limited to acute injuries, traumatic incidents, mental health and addiction challenges, palliative diagnosis, cognitive decline, systematic barriers, etc. If you have a passion for providing patient-centered care then we would like to meet you!


• Perform psychosocial assessments for the clients of the Health Centre, in all clinical areas and for all presenting situations, as requested or identified by high risk screening;

• Provide and support a culturally safe practice within all aspects of the role at NSHN;

• Intervene efficiently in a variety of presenting situations according to the treatment goals and plan;

• Offer social work services according to qualifications;

• Represent the organization at community tables and participate/chair some NSHN committees;

• Lead QI/QA initiatives with a focus on improved patient care;

• Other duties as assigned.


• Current registration in good standing with the Ontario College of Social Workers and Social Service Workers;

• Bachelor of Social Work;

• Masters of Social Worker considered a strong asset;

• 4 to 5 years in a healthcare field for a candidate prepared at the Bachelor of Social Work level;

• Gentle Persuasive Approaches, Level 1 considered a strong asset;

• Non-Violent Crisis Prevention Intervention Training considered an asset;

• Demonstrated ability to work as a committed team player;

• Ability to exercise good judgment, show initiative, and is proactive;

• High standards of ethics and confidentiality to handle sensitive information;

• Knowledge of Microsoft Word, Excel, Power Point and Windows;

• Excellent interpersonal and communication skills, both written and verbal;

• Fluently bilingual (English & French) considered a strong asset.
